Full Time: 37.5 hours a week - mainly 8:30-4:30, Monday -Friday, may be times that you will work outside of these hours. Primary Locations: This role will be providing services to different locations in Lunenburg and Queens County. Position Scope - A Case Manager assesses an individual’s employment/career needs and identifying those who are suitable for case management. This role works collaboratively with clients to support them in developing a Return to Work Action Plan (RTWAP) and to address the specific goals as documented by the client. Those who provide case management support accountability by organizing service delivery that is appropriate, coordinated, and timely to address client needs and desired outcomes. They can define and manage career and personal development goals and transition and have a solid understanding of counselling assessment processes, tools and resources. The ideal candidate is client-focussed, practices from a strengths-based lens and is passionate about the career development process. Main Duties and Responsibilities:
